If you are interested in trying out for the Girls or Boys basketball program for the upcoming season.  You must have a current and up to date physical.  This means you must be seen by a doctor and have the PROPER PSAL PAPERWORK COMPLETELY FILLED OUT!!!!  I have attached all the paperwork you will need to have signed by you, your parent/guardian, and the doctor.   YOU MUST USE THE FORMS I HAVE ATTACHED.  Student Athletes must also be vaccinated to participate in high-risk sports.

Paperwork will be collected the week of October 17th .  No Cut tryouts will begin shortly after.  If you are unable to see your regular physician:

1. CityMD ( You must have your insurance card and all paperwork signed, if you are under the age of 18 you must go with a parent/guardian.)
2. MiDoctor  located on 49th and 9th Avenue  ( You must have your insurance card and all paperwork signed, if you are under the age of 18 you must go with a parent/guardian)

YOU CAN NOT TRYOUT WITHOUT A PHYSICAL
